IOC:
A Community-Based Service Delivery Model
Improving Outcomes for Children (IOC) enables DHS to work more closely with the communities that our children come. It is our goal to create a service system informed by and that speaks to, the needs of those communities.
DHS intends to achieve four outcomes with the implementation of IOC:
1. An increased number of youth are safely in their own homes and communities while their families receive services;
2. An increased number of youth will achieve timely re-unification or other measures of permanence;
3. Utilization of congregate care will decrease; and
4. Child, youth and family functioning will improve
Through this service system provider agencies, referred to as Community Umbrella Agencies (CUA's) deliver case management services and support to families and children. This population is involved in the child welfare system within 10 geographical jurisdictions aligned with the city's police districts. This community model has a strong focus on Family Teaming which brings together the family, service and system partners at key decision points over the life of the child.
